1. VOGUE Dental offers value-added service to its members including, but not limited to providing free consultation to its members and their families, sending sporadically updated information on the activities and promotions by post.
2. Please see the card type on the attached form in the bottom.
3. Any type of cardholders needs to provide genuine personal information upon the first visit.
4. Notice for cardholders:
1. Follow Vogue's regulation on use of VOGUE membership card.
2. Please show your VIP Card upon each visit.
3. One member card is allowed to use for each spending.
4. VIP card cannot be used in conjunction with any other offers, special discounts or promotions.
5. Please inform VOGUE  Dental in time when you change your contact address.
6. VOGUE Dental will not refund if you want to terminate your membership without justified reason.
5. Usage of VIP Card
1. Cardholders need to make sure that there is enough prepaid amount by the mildlink POS machine before a treatment. And please confirm and sign on the printed receipt after a treatment.

      The terms and conditions must be validated by vogue dental's stamp
      before becoming effective. If there is any ambiguity in the terms and
      conditions, the final interpretation is due to vogue dental, the usage of VIP       card is subject to the terms in the related contract.
